In Intimations of Mortality, Barbara Reich offers an empirically-based critique of the failures of end-of-life communication and decision-making in the United States. Using England and Canada as occasional foils, Reich explores why U.S. physicians, patients, and families struggle to have the conversations necessary to provide seriously ill and dying patients with medical care consistent with their preferences. Reich also shows how a number of different factors -including payment mechanisms, liability fears, cultural phenomena, communication avoidance, death denial, and clinical uncertainty -impact physician-patient communication and medical decision-making, leave patients and families without the tools they need to make informed choices, and instead leave the default practices in place. The pervasive influence of law on medical practice and clinical bioethics is often noted with a combination of exasperation and lamentation. Physicians and non-physician bioethicists, generally speaking, consider the willingness of courts, legislatures, and regulatory agencies to insinuate themselves into clinical practice and medical research to be a distinctly negative aspect of contemporary American society. They are quick to point out that their colleagues in other Western developed nations are not similarly afflicted, and that the situation which obtains elsewhere is highly preferable to the legalization and purported over-regulation of medicine that has taken place in the United States during the last fifty years. In this book I offer a decidedly different perspective. It is, admittedly, not entirely without personal and professional bias. Prior to becoming a fu- time academic, teaching bioethics in the setting of an academic medical center, I was, for nearly 20 years, an attorney specializing in health law. Even after earning a doctorate in philosophy, I was frequently considered to be the "resident lawyer" on the bioethics faculty, much more frequently looked to for my insights on the law than my perspective as one who had formally studied moral philosophy and applied ethics. I note this not out ofa sense of frustration or disappointment, but as confirmation that even among physicians and n- physician bioethicists, there is widespread recognition that the law does have important contributions to make in assessing the practice ofmedicine and the conduct of medical research.

Effective control of breast cancer depends on three types of research accomplishment -- understanding the disease's origins and progression: successfully applying this knowledge to methods of detection, diagnosis and treatment: and finding ways to make these advances truly available to the public as effectively as possible. The significant progress that is occurring across this entire spectrum of pioneering investigation is reflected in these proceedings of the 1987 biennial conference of the International Association for Breast Cancer Research. The first section of the book focuses on oncogenes and chemical effectors that may play key roles in early cell transformation leading to breast cancer. Research discussed includes identification of specific oncogenes which appear to be involved in the disease, study of their activation and expression, examination of the biological effects of various growth factors isolated from breast cancer cell lines, and investigation of the molecular mechanisms by which estrogens promote and stimulate growth of breast cancers. The second group of chapters deals with several other complex factors and phenomena which may influence tumor formation in the breast, for example, expression of abnormalities by fibroblasts, disruption of epithelial-mesenchymal interactions, and loss of ability nili to synthesize normal basal lamina resulting in alterations in the extracellular matrix. Clarification of the processes of normal mammary gland development and differentiation is central to much of this work.

We stand today on the threshold of a new understanding of cancer. Primarily through the powerful tools of molecular biology, unified hypotheses explaining the origins of the disease are emerging and rapidly being validated. This volume, which presents the latest findings from laboratories throughout the world on the role of RNA tumor viruses in cancer, is a celebration of these achievements and a prediction of further progress leading ultimately to the control of the disease. It is important in this context to recall the natural history or life cycle of RNA cancer virology. From the earliest days of the science, when viruses were first recognized as distinct biologic agents of etiologic significance, their role in cancer was proposed and hotly debated. The critical early discoveries, even those made as recently as 25 years ago, were met with rejection; not skepticism or cautious restraint, but outright rejection. During the 60's, there was a gradual acceptance of the association between viruses and cancer, the result of landmark studies in experimental systems, and this led to a frenzy of activity in the field. There followed another period of doubt and uncertainty, due to the difficulty in attempting to apply directly, and in retrospect inappropriately, the tenets of infectious disease to human cancers, only to have the field resurrected, revitalized and redirected by the explosion of progress in molecular biology and genetics.

In breast cancer as in other cancers on the front line of modern interdisciplinary research we have crossed the threshhold of new understanding. Fueled by an awareness that breast cancer is a leading cause of morbidity and mortality in women and honed by rapid technological advances in molecular mechanisms, major inroads into the darkness of this scourge have been accomplished during the past several years. Basic laboratory and clinical research findings must influence and in turn be influenced by efforts to detect, to diagnose and to cure the human disease. It is indeed gratifying to note that the efforts of scientists and clinicians from throughout the world reported in this volume are achieving this objective. The first section of the book focuses on the molecular and genetic basis of breast cancer. The role of specific oncogenes in mammary tumorigenesis using transgenic mice and mammary glands correlating molecular events with specific stages in neoplastic development are described and discussed. Such topics as the nature of specific oncogenes, levels of oncogene expression, the alteration of expression of other growth regulatory genes and the state of the cell in which the oncogene is expressed are specifically addressed. This section of the book is rounded out with discussions on the potential of genetic alterations as indicators of prognosis, the characterization of full-length cDNA codes for breast cancer markers and the function of antigens in tuomorigenesis.

The control of breast cancer, a leading cause of cancer death in women, will depend ultimately on our understanding of the disease--its origin, and progression which in turn will permit the effective management of its treatment, its detection, and perhaps even its prevention. This book takes us inside the Child Protective Services, for an in-depth look at the entire organization. Examining the role of the agency from the initial dealing with a family, to the end when a case is discharged, the author shows how parents negotiate with the state for custody of their children and how being held accountable to the state affects these families. Within each chapter are heartbreaking stories culled from the author's ride-alongs with social workers, or the numerous juvenile court cases that she was able to observe -- stories which illustrate the personal affects of bureaucratic decisions.

Medical school taught John Rich how to deal with physical trauma in a big city hospital but not with the disturbing fact that young black men were daily shot, stabbed, and beaten. This is Rich's account of his personal search to find sense in the juxtaposition of his life and theirs. Young black men in cities are overwhelmingly the victims -- and perpetrators -- of violent crime in the United States. Troubled by this tragedy -- and by his medical colleagues' apparent numbness in the face of it -- Rich, a black man who grew up in relative safety and comfort, reached out to many of these young crime victims to learn why they lived in a seemingly endless cycle of violence and how it affected them. The stories they told him are unsettling -- and revealing about the reality of life in American cities. Mixing his own perspective with their seldom-heard voices, Rich relates the stories of young black men whose lives were violently disrupted -- and of their struggles to heal and remain safe in an environment that both denied their trauma and blamed them for their injuries. Two of the questions most frequently asked by archaeologists of sites and the objects that populate them are 'How old are you?' and 'Where are you from?' These questions can often be answered through archaeometric dating and provenance analyses. As both archaeological sites and objects, shipwrecks pose a special problem in archaeometric dating and provenance because when they sailed, they often accumulated new construction material as timbers were repaired and replaced. Additionally, during periods of globalization, such as the so-called Age of Discovery, the provenance of construction materials may not reflect where the ship was built due to long-distance timber trade networks and the global nature of these ships' sailing routes. Accepting these special challenges, nautical archaeologists must piece together the nuanced relationship between the ship, its timbers, and the shipwreck, and to do so, wood samples must be removed from the assemblage. Besides the provenance of the vessel's wooden components, selective removal and analysis of timber samples can also provide researchers with unique insights relating to environmental history. For this period, wood samples could help produce information on the emergent global economy; networks of timber trade; forestry and carpentry practices; climate patterns and anomalies; forest reconstruction; repairs made to ships and when, why, and where those occurred; and much more. This book is a set of protocols to establish the need for wood samples from shipwrecks and to guide archaeologists in the removal of samples for a suite of archaeometric techniques currently available to provenance the timbers used to construct wooden ships and boats. While these protocols will prove helpful to archaeologists working on shipwreck assemblages from any time period and in any place, this book uses Iberian ships of the 16th to 18th centuries as its case studies because their global mobility poses additional challenges to the problem at hand. At the same time, their prolificacy and ubiquity make the wreckage of these ships a uniquely global phenomenon.
